This vaccine is not required. If you choose to do this vaccination, it needs to updated once yearly. It CANNOT be given within 5 days of the start date of boarding as your dog can become a carrier of bordetella and will not be fully immune for 3-5 days. For the safety of your dog, please leave plenty of time before boarding to have this updated.

For the safety of your dog and the other dogs in our care, please read the following terms and conditions of boarding with Canine Harmony.
DEPOSITS:
Now in effect all bookings require a non refundable deposit to complete your dog's booking. Deposit amounts are determined by how many nights you have booked for.
Deposits can be made by e-transfer, credit card over the phone, paid by card in office or on our
website
. Deposits are non-refundable and will be forfeited if there is a cancellation. Deposits cannot be transferred to another booking but can be moved if you are rescheduling your dates. Your Deposit will be used towards your final bill.

PAYMENTS:
Payments for boarding reservations are now due upon drop off / at the beginning of your stay. Payment must be made in full, and the deposit amount will be deducted from the total at this time. If you arrive back early, you can receive a store credit for any additional days you've pre-paid.
CANCELLATIONS:
Cancellations to a reservation will result in forfeiting your deposit. We require at least 48 hours notice for Cancellations without any further penalty. If you cancel ib less than 48 hours of your reservation you will be required to pre-pay for any future bookings with Canine Harmony. If you do not show up for your reservation you will be required to pay for the full amount for the reservation before booking with us again.
No Returns or Refunds on any services. All Sales are final.
WAIVER
: By submitting this application and agreeing to the following terms and conditions, I agree that my dogs has not been exposed to parvo, distemper, rabies or any other contagious disease or parasite in the last 30 days. I will inform Canine Harmony if my dog has an eye infection, runny nose, vomiting or diarrhea prior to this visit.
I understand that attendance at a dog boarding facility is not without risk to my dog, because some of the dogs to which my dog will be exposed may be the cause of injury even when handled with the greatest amount of care.
I hereby waive and release CANINE HARMONY DAYCARE & BOARDING herein after referred to as the Organization, its employees, officers, members, and agents from any and all liability of any nature, for injury or damage which my dog may obtain, including specifically, but without limitation, any injury or damage resulting from the action of any dog, and I expressly assume the risk of such damage or injury while my dog attends dog boarding, dog daycare, or while on the grounds or the surrounding area.
In consideration of and as inducement to the acceptance of my application for Dog Daycare/Boarding by this Organization, I hereby agree to indemnify and hold harmless this Organization, its employees, officers, members, and agents from any and all unavoidable accidents, while on the grounds or the surrounding area thereto as a result of any action by any dog, including my own, as well as any accident which may occur while on the grounds or surrounding area.
Canine Harmony Daycare & Boarding agrees that, while the dog is on the premises and under its control for boarding and daycare, we will exercise the UTMOST DILIGENCE and care to protect the HEALTH AND SAFETY of the dog. However, the owner of the dog agrees that Canine Harmony Daycare & Boarding has no liability for loss or damage to the dog or the owners property resulting from UNAVOIDABLE ACCIDENTS such as fire, natural disaster or other unavoidable acts of nature.
In case of accident, injury or other medical problem, I allow Canine Harmony to administer any treatment necessary to protect the health and safety of my dog while on the premises.
Dogs may be energetic and wrestle or jump on each other during group play. Your dog may get a scratch from time to time if they play with other energetic dogs. Some breeds that are more prone to scratches are dogs with very short hair and little undercoat such as boxers, bulldogs, Dobermans and doodles with short hair cuts. The employees at Canine Harmony who are certified in Canine First Aid will address any scratches and will be checked, treated and monitored as needed. Vet care is typically not necessary for a minor scratch.
In case of accident, injury or other medical problem to my dog(s) that Canine Harmony is unable to treat on the premises, I authorize Canine Harmony to take my dog to my own vet or any other available vet including an Emergency Veterinarian (if it is after hours) to obtain the necessary treatment required for the health and safety of my dog. Canine Harmony will attempt to contact me, but if I am unable to be contacted, necessary vet care will be provided without my permission. The health and safety of the dog is our primary concern.
I agree to pay all vet bills that my dog incurs while under the care of Canine Harmony and any other costs associated with said vet visit. If my dog causes injury to another dog while at Canine Harmony I agree to pay all vet bills
associated with the incident for the other dog.
If, at the completion of the agreed boarding time, the dog is not picked up by the owner and payment of all charges made in full, including applicable medical, veterinarian or boarding charges as provided herein, Canine Harmony Daycare & Boarding shall notify owner by certified mail at the address shown above that the dog will be held for an additional fourteen (14) business days following the date of mailing the notice. At the end of the fourteen (14) day period, if there is no response from the owner, the dog will be deemed abandoned. Abandoned dogs will become the property of Canine Harmony and will be adopted out at the discretion of Canine Harmony to a suitable home. The proceeds from said adoption will be applied to the charges of the boarding invoice.
Should the owner return for the dog after being deemed abandoned, the owner may at the discretion of Canine Harmony obtain the dog back, but will be charged for any additional days and food required to board the dog at Canine Harmony at the current daily boarding rate.
Owner represents that he or she is the legal owner of the dog(s); that the dog(s) is/are free and clear of any mortgage or encumbrance; that the dog(s) has/have been vaccinated as shown above and that the owner has read this agreement in its entirety.
